Medicare Coverage Policy

for Diabetic Shoes and Inserts

1.  Patient must have enrolled with Medicare Part B

2.  Patient, who is diagnosed with Diabetes, must have a physician to authorize the use of diabetic shoes.

3.  Patient also must one or more of the following medical conditions, to qualify:

a. Foot deformity

b. Peripheral neuropathy with evidence of callus formation

c. History of pre-ulcerative callus

d. History of previous foot ulceration

Medicare Coverage Policy given above may alone may not qualify a patient for diabetic shoes.  There may be additional requirements by the insurance company to qualify for the products and services.
